Fundraising, payment processing and prize promotions are regulated, and the rules differ by state and country. Text in [SQUARE BRACKETS] is a fact that has not been filled in. Have counsel review and complete this document before accepting real donations.
Each campaign's prize promotion is run by that campaign's organizer, not by Unlock for Good. This page is the template those rules are built from. Where a campaign publishes its own official rules, those rules govern and this template does not override them. NO DONATION OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN. A DONATION WILL NOT IMPROVE YOUR CHANCE OF WINNING. VOID WHERE PROHIBITED.
1.Sponsor
The sponsor of this promotion is the campaign organizer: [SPONSOR LEGAL NAME], [MAILING ADDRESS]. Unlock for Good provides the technology platform only and is not the sponsor, promoter or administrator of the promotion.
2.No donation necessary
A donation is not required to enter and will not improve your chance of winning. Entrants who take the free route and entrants who donate receive the same number of entries — one — and their entries are treated identically in the drawing. Making a larger donation does not produce additional entries.
3.How to enter — free method of entry
Free entry method A: open the campaign's challenge link, start the unlock timer, wait the stated period, and unlock the campaign without donating. This receives one entry wherever the campaign is open to all participants. Free entry method B (required where a campaign restricts automatic entries to donors): hand-print your full name, complete mailing address, email address, date of birth and the campaign name on a plain 3" x 5" card and mail it in a stamped envelope to [AMOE MAILING ADDRESS]. Mail-in entries must be postmarked by [POSTMARK DEADLINE] and received by [RECEIPT DEADLINE]. Limit one mail-in entry per person per campaign. Mechanically reproduced, photocopied or bulk entries are void. Mail-in entries receive the same weight as any other entry. An organizer who restricts automatic entries to donors must publish a working free method of entry before the promotion opens.
4.Entry period
The promotion begins at [START DATE AND TIME] and ends at [END DATE AND TIME], [TIME ZONE]. Entries submitted before or after the entry period are void. The sponsor's clock is the official timekeeper.
5.Eligibility
Open to legal residents of [ELIGIBLE JURISDICTIONS] who are [MINIMUM AGE] or older at the time of entry. Employees, officers and directors of the sponsor, and their immediate family and household members, are not eligible. Void in [EXCLUDED JURISDICTIONS] and wherever prohibited or restricted by law. Limit one entry per person per campaign. To enforce this, an entry is only recorded once the entrant confirms their email address by following a link sent to it; unlocking the campaign does not require confirmation, and an entry owed is awarded as soon as the address is confirmed. Entries from anyone found to have used multiple identities, automated methods, or any means of circumventing these rules will be disqualified.
6.Bonus entries for referrals
Where a campaign offers them, a participant may earn additional entries when a stated number of people they referred complete a donation. Bonus entries are awarded once per participant per campaign and are described on the campaign page before entry. [Counsel to review: this ties additional entries to the conduct of third parties who paid. Some jurisdictions treat referral-contingent entries as consideration. Confirm this structure is permissible in every jurisdiction offered, or restrict or remove it.]
7.Prize

The prize is awarded as-is with no warranty, is not transferable, and no substitution or cash equivalent is permitted except by the sponsor, who may substitute a prize of equal or greater value if the stated prize becomes unavailable. All costs not expressly listed are the winner's responsibility.
8.Odds and winner selection
Odds of winning depend on the total number of eligible entries received. Winners will be selected in a random drawing from all eligible entries on or about [DRAWING DATE], conducted by the sponsor using the drawing tool provided by Unlock for Good. The drawing is final.
9.Winner notification and claim
Winners will be notified by email at the address given on entry within [NOTIFICATION WINDOW] of the drawing. A winner must respond within [CLAIM WINDOW] and may be required to sign an affidavit of eligibility, a liability release and, where lawful, a publicity release before the prize is awarded. If a winner cannot be reached within the claim window, declines the prize, is found ineligible, or fails to return required documents, the prize may be forfeited and an alternate winner selected at random.
10.Taxes
All federal, state and local taxes on a prize are the sole responsibility of the winner. Where the value of a prize requires it, the sponsor will issue an IRS Form 1099 and may require a completed Form W-9 before awarding the prize.
11.Publicity
Except where prohibited, accepting a prize constitutes permission for the sponsor to use the winner's name, city and state for promotional purposes without further compensation. Residents of jurisdictions where this is not permitted may win without granting it.
12.General conditions
The sponsor may disqualify any person who tampers with the entry process or the operation of the promotion, or who acts in a disruptive or unsportsmanlike manner. If the promotion cannot run as planned because of technical failure, fraud, or any cause beyond the sponsor's reasonable control, the sponsor may suspend, modify or terminate it and award prizes from eligible entries received up to that point. By entering, entrants release the sponsor and Unlock for Good from any liability arising from participation or from acceptance or use of a prize, to the fullest extent permitted by law.
13.Disputes

14.Registration and bonding
[Counsel action required before any promotion opens: a sweepstakes with a total prize value above $5,000 generally requires registration and bonding in New York, and above $5,000 in Florida, with filing deadlines before the promotion starts. Rhode Island has retail registration requirements. Confirm what applies to each campaign, or restrict prize value and eligible jurisdictions accordingly.]
15.Winners list
For the name of the winner, send a stamped, self-addressed envelope to [WINNERS LIST ADDRESS] within [WINNERS LIST WINDOW] after the drawing date.
